PTO Payment Request Form
Please use the form linked below to request eligible payments from PTO funds. Before processing, all payments must be approved by both an Event Committee and a PTO Board Member.
- Approval Required: Payments must be approved by an Event Committee and a PTO Board Member prior to processing.
- Processing Time: Please allow up to two weeks for processing.
- Urgent Requests: For urgent requests, please contact the PTO Treasurer directly.
